They will carry something akin to a phantom wound. Other PCs and some NPCs will be able to see it, but others will not. They will have to get medical care from someone who can actually see the wound in order for it to heal. If left untreated, characters will find these wounds fade slower than anything natural and they will be aware of a pitch black scar on their body, even if it's not fully visible to others.
